2018 Nutrition for Life Healthcare Individual and Dual Appointment Policy

Nutrition for Life Healthcare want to help you maximise your health potential. It is with our duty of care that we take your healthcare plans with us seriously.

As a valued client we work with you to mutually agree on the appointment plan which will best suit your needs and goals. However here are a few things we want you to read in relation to our clinic appointment policies.

Rescheduling

We have a 2 week reschedule policy; this means that if you need to reschedule we encourage you to make your next appointment within 2 weeks from the appointment you are changing. Outside of this time-frame, we may charge a reschedule fee of $20 added to your next appointment fee. Extensive reschedule periods of more than 6 weeks will be considered under an appointment cancellation and details of this are below.

Cancellation

Cancellation of your appointment will incur a 50% charge of the appointment type. This applies to appointments rescheduled more than 6 weeks from your previous appointment time and for appointments you are cancelling. Cancellations made with less than 24 hours notice incur a 100% charge of the appointment time that was to be attended.

Missed Appointments

Missed appointments without any notice or late communication (less than 24 hours) will incur a 100% charge of the missed appointment fee.

We understand unforeseen circumstances do arise, but please contact us if you need to make any changes to your appointment booking as soon as possible.
